What is HERO Indian Initiative?
H.E.R.O. Indian Initiative:
Health Education Resources & Outreach (H.E.R.O.) Indian Initiative is established to address multiple health disparities and diseases detrimental to the Indian Nations by providing health and nutritional education, tools and resources, clinical and pharmaceutical services from minimal to no cost.
Mission:
In order to reduce or eliminate from the Indian Nations the overwhelming disparities in chronic diseases such as diabetes, heart disease, cancer and cognitive/behavioral issues by providing the appropriate tools to manage chronic diseases, and prevent future health disparities.
Vision:
H.E.R.O. Indian Initiative will partner with the Indian Nations and Tribal Leaders members are at highest risk for developing diabetes and related chronic disease by providing unique clinical services at affordable prices to address their healthcare needs.
Who HERO Indian Initiative Is
A nationally recognized accredited diabetes curriculum through the American Diabetes Association of Diabetes Educators and Indian Health Services.
An interactive team approach where everything is customized to the needs of the tribal members and they are in control.
Practice evidenced based practices and guidelines from the American Association of Diabetes Educators (AADE), the American Diabetes Association (ADA) and Indian Health Services (IHS)
UNIQUE! HERO Indian Initiative diabetes program is unique in that it delivers its products and services in locations that are easily accessible by the tribal community.
While basic information regarding diabetes self-management education is the same, our presentation is directed to accommodate the tribal audience.
The curriculum offers tribal members with diabetes the tools needed to solve general problems, set goals, and empower them to make healthy lifestyle choices and effect positive outcomes.
We operates under the premise that an educated tribal community is more informed to make wiser decisions concerning their health and improved outcomes.
